This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental Services:  Comprehensive dental exams  Emergency treatment  Fillings  Extractions  Root canals  Crowns  Dentures  Partials  Cleanings  Sealants  Gum treatments  Fluoride treatments   A Sliding Fee Discount is available to all patients based on family size and income up to 200% of the Federal Poverty Guidelines. An application for the Sliding Fee Discount must be provided annually by each patient or family with proof of income.  Hours:  Monday, 9:15 am - 5:30 pm. Tuesday - Friday, 8:15 am - 4:30 pm.
