Broward County Health Department Children's Dental - Children's Diagnostic Treatment Center Hours of operation:  Thursday 8am - Noon, 1pm-5pm Fees charged:  Medicaid, Self-Pay/Full, Self-Pay/Sliding scale (families whose income is below 200% Federal Poverty Level) Intake procedure:  Appointment, proof of income, social security card, tax statement, parental consent to treat and residence address required Eligibility requirements:  Must be between 2.5-20 years old, client of Children Diagnostic & Treatment Center and/or diagnosed as HIV positive Languages spoken or available:  English Area served:  Broward County Clinical preventive and restorative dental services for children between 2.5 to 20 years old from families whose income is below 200% the Federal OMB poverty level; program's goal is to improve the dental health of the youth of Broward County.
